IODP Membership Fee

$399 – Adult members 14 years old and over

$299 – Children 13 years old and under

All IODP membership fees are due and payable at the time of registration and are non-refundable when services have been provided. The plan duration is one year from the registration date. ALL patient portions for services received are due at the time of services to receive IODP benefits. Please notify our office at least 48 hours in advance if you must change your reserved appointment.

Membership Savings Plan Limitations and Exclusions

Our In-Office Dental Plan aims to provide affordable dentistry to the Greenwood community. You can receive significant dental savings on most treatments and procedures you require to maintain a healthy, brilliant smile. However, kindly note that IODP has limitations and exclusions, including the following:

• Any medications – including pulp caps, fluoride treatments, IV sedation, non-IV sedation, nitrous oxide, etc.
• Any orthodontic treatment
• Any procedure that is considered experimental
• Hospitalization for any procedures
• Services unable to be completed due to the patient’s medical health, mental health, or other unhealthy status
• Conditions or services under Worker’s Compensation or Employer’s Liability Laws
• Congenital malformations other than congenital anomalies of the tooth/teeth from birth
• Any diagnosis or treatment of myofascial pain dysfunction syndrome
• Any procedures for full mouth rehabilitation requiring appliances or restorations
• Any alterations, restorations, or treatments of the temporomandibular joint
• Any services provided for free by a county, government, municipality, or other agency
• Any appliances, diagnosis, or treatments conducted by a referral made to another dentist or specialist outside of the providers of our office to complete treatment in connection with any dental procedure in this office
• IODP patients cannot use their own dental insurance benefits or other dental coverage in conjunction with any part of the IODP membership
• Any unused services within the IODP membership are nontransferable to other plan members, regardless of the service
• Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a 25% fee for the services that were to be rendered
• No Refunds
• Periodontal disease patients are eligible to receive a periodontal maintenance cleaning, provided within the membership plan, twice a year. However, this is not sufficient cleaning for a patient with periodontal disease. The cost for such additional cleanings shall be paid at the time of service rendered.
